I think Simmonds may have been heading for a yellow there until Hepburn got involved as Simmonds seemed to lift a leg and twist Marler, potentially pulling his arm away. Then Hepburn joined in and drove him into the ground.

The problem for the ref is hearing the call. The player could always claim they called it and repeated themselves louder. Putting your hand up in the air can throw you off balance and makes you vulnerable to a hit on the ribs, so you tend to put the hand up after you are sure you are 'safe', so typically a couple of steps and seconds.

Whilst I accept some of that both today and yesterday you could see when they shouted mark and put their arm up. It was not immediate by any means.
